2) Search & Complete the Ordering Guide online
- Using the search function, you can find the prescription(s) you need by:
- Typing in the name of the medication or
- Selecting the first letter of your medication in the drop-down box
- To add a medication, just click the "add to cart" button.
- You can now search for more medications, or proceed to checkout.
- Once you have finished the quote and clicked on "checkout", simply fill in the required information.
- You can then receive the Ordering Guide using 3 different methods:
- Print the ordering guide immediately
- Have the ordering guide faxed to your fax number
- Have the ordering guide mailed to your home/work address
- Once you've obtained the ordering guide, please complete the remainder of the form.
- Fax or mail the completed ordering guide as indicated (NOTE: please don't forget to include your
doctor's prescriptions).
- Allow 7 to 15 business days for delivery from the time we receive/verify
your ordering guide.
